ARE YOU OR A LOVED ONE
ELIGIBLE FOR HOSPICE?
To be admitted to Oak Hospice and Palliative Care, patients can meet the criteria based on their diagnosis or symptoms. You have the option to either contact us directly via email and/or phone call OR complete the referral form available on our website under the "Make A Referral" tab.
Some possible signs that a person may be ready for hospice include (but not limited to):

restlessness or repetitive motions

decrease in food and fluid intake

loss of urinary or bowel control

increase in the amount of time spent sleeping

changes in body temperature (fever or feeling cool to the touch)

chest congestion and/or rapid breathing

uncontrolled pain, nausea, or vomiting

frequent hospitalizations or trips to the emergency room

experiencing frequent or recurring infections

inability to perform daily tasks without assistance

rapid decline in health despite aggressive medical treatment
